Permission Requests from Authors of Medical or Scientific Articles or Book Chapters

Permission requests are considered for reproduction of Text, Tables, or Figures to the authors of abstracts, articles or chapters of scientific meetings, medical journals or books. The Commission on Cancer of the American College of Surgeons copyrights all Text, Tables, and Figures featured in the NCDB Benchmark Reports and NCDB Survival Reports. Permission is granted on a limited, non-exclusive basis for both print and electronic one-time use only for the life of one edition with no use in any derivative or ancillary works.

Should you wish to reproduce any Text, Table, or Figures from these reports in a medical or scientific meeting abstracts, journal articles or book chapters please complete the following "Permission Request Form for Individuals" the "Terms and Conditions Statement" and mail request to The American College of Surgeons, Commission on Cancer; 633 N. Saint Clair Street, Chicago, IL 60611.

If your request is granted, a credit line will be provided and this credit line must be included where the material is printed.

Reasonable permission requests are granted routinely. All requests will be processed within 10 to 14 business days.

All fields must be completed and a dated and signed copy of the "Terms and Conditions Statement" must be submitted to the NCDB before your form may be processed. Completed forms may be transmitted via fax at (312) 202-5009. If you have questions, please contact NCDB staff via e-mail at ncdb@facs.org.

A copy of the final printed material must be forwarded to NCDB staff.
